MEDICAL INTAKE & TREATMENT CONSENT

Before receiving services, you must complete a medical intake form and sign a treatment consent.

You agree to:

Disclose all relevant medical history, medications, allergies, and previous aesthetic treatments
Follow all pre- and post-treatment instructions
Update us immediately if your medical condition changes

We reserve the right to refuse or modify treatment if it is not medically appropriate or safe.

Aesthetic medicine is not an exact science. By receiving treatment, you acknowledge that:

Individual results vary
No specific outcome is guaranteed
More than one session may be required to achieve desired results
You are financially responsible for all services rendered

Photographs may be taken before and after treatment for medical documentation.

PAYMENT & REFUND POLICY

Full payment is due at the time of service.
We accept all major credit cards and offer third-party financing (ex: PatientFi).
All services, packages, memberships, and deposits are non-refundable.
Pre-purchased packages must be redeemed within 1 year of purchase unless otherwise stated.
Retail skincare products may be returned within 14 days if unopened and unused.

MEMBERSHIPS & CREDITS

Membership details (including Skin Bar Society, Beauty Bank, Skinvestment packages, etc.) are outlined separately in your membership agreement.

By joining a membership, you agree that:

Membership fees and credits are non-refundable
Credits expire at the stated term
Memberships cannot be transferred or shared
Failure to follow payment terms may result in suspension or cancellation

GIFT CARDS

Gift cards are:

Final sale
Non-refundable and non-transferable
Redeemable only for services or retail at Maintenance Bar

Gift cards do not expire, unless required by state law.

WEBSITE & DIGITAL CONTENT

By using our website, you agree not to:

Misuse or disrupt site functionality
Copy, distribute, or reproduce content without permission
Submit false or misleading information

All content—including images, text, branding, and design—is protected by copyright and intellectual property laws.

COMMUNICATION CONSENT (SMS & EMAIL)

By providing your email or phone number, you consent to receive:

Appointment confirmations and reminders
Treatment updates
Promotional messages and newsletters

You may unsubscribe at any time. Message and data rates may apply.

HEALTH & SAFETY

We maintain strict sanitation, sterilization, and safety protocols. You agree to:

Follow post-care instructions
Notify us of any concerns or complications promptly
Seek emergency care for urgent medical issues

RIGHT TO REFUSE SERVICE

We reserve the right to refuse service for:

Safety concerns
Inappropriate or abusive behavior
Failure to follow policies
History of no-shows or noncompliance

LIABILITY

By receiving treatment, you release Maintenance Bar and its providers from liability for:

Normal risks inherent to cosmetic procedures
Reactions or complications that occur due to undisclosed medical history
Failure to follow aftercare instructions

This does not waive liability for gross negligence or misconduct.

CHANGES TO TERMS

We may update these Terms of Service at any time. Updates will be posted on our website with the revised date.
